Time is the most important issue. There is a "golden hour" philosophy in EMS that dictates quick care and transportation of a patient. Some serious conditions or injuries are not as time sensitive as others, but most are and EMS and some ERs don't always have the capability to distinguish between them. So they simply treat most serious calls the same as far as the emergency of it.
Availability of a chopper and crew is another. Where the patient needs to go is another and as I said before, the weather can play a major part in whether one agency can fly or not and if another needs to be called. There could be hundreds of factors on any given day that can either make or break medivac operations.
I don't know your situation, but if you're sick enough or seriously injured enough to necessitate a flight in an air ambulance, you should be happy that they were available and conditions were favorable enough to fly you to a proper facility.
If I were put in that position, I'd count my blessings instead of my money.
